This registration form is for ONE person only. A separate expression of interest to attend services must be completed for each individual.

That within the last 14 days, I have not visited an area which has been identified as having increased COVID-19 activity that would require you to self-isolate or be tested under local government guidelines, as outlined by the NSW Government website https://www.nsw.gov.au/covid-19/latest-news-and-updates#latest-covid-19-case-locations-in-nsw
I have not experienced any flu-like symptoms, including any of the following within the last 14 days:
Fever (37.5+), cough, running nose, sore throat, nausea or vomiting, diarrhea, sore throat, chills
I have not had contact with a confirmed or suspected case of COVID-19 in the last 14 days;

* I consent to the North Shore Temple Emanuel and its staff collecting, storing, using and disclosing my personal information (including health information) for the purposes of assessing my application, and disclosing my name, address and contact details to any government health official on request so that I can be contacted promptly if it should transpire that someone who has tested positive to the COVID-19 virus attended the same service. (Note: NSTE intends to retain the information until the government declares an end to the risk of COVID-19 infection by community spread, after which all record of such information will be destroyed).
